What is Advance Care Planning (ACP)
Advance Care Planning is a process of making decisions about the kind of care you would like to have in the future. Although we would like to think we will have the mental capacity to make these decisions at the time, when they are required, this is not always the case.
By completing this ACP booklet, you will have the opportunity to make your wishes known about your care. Writing down your wishes can make it easier for your loved ones and health professionals to consider your needs if you are not able to voice them.

Once you have decided what to have in your plan, give a copy to your loved ones, your GP and let us know. If you are under our care we will keep a copy in your electronic notes.
