Hospice at Home enables patients to receive specialist care and support provided by ellenor in familiar surroundings. This includes:
When we have received a referral, the patient will be contacted to arrange an appointment for an assessment with the most appropriate member of the team. Depending on circumstances, an outpatient appointment may be offered or a home visit.
We work closely with GPs, District Nurses, and other community services. We will not replace or take over the care provided by other services, but will add additional resources to help make things better for patients and families. All staff have training and experience in the provision of practical, appropriate and sensitive end of life care, in the home setting, and are appropriately supported and supervised.
Support for patients in Care Homes
For patients living in a care home, we have a team who work alongside Care Home staff to offer support, guidance and training, in order to enable residents to die in comfort and with dignity in familiar surroundings and to avoid inappropriate hospital admissions.

Our crisis support service provides a rapid response in a crisis situation for people at the end of life (where the family / carers require additional support to avoid a breakdown which would otherwise lead to a hospital admission). We also provide practical hands-on care and emotional support in the home by suitably trained staff, available 24/7. This service allows individuals to die in their preferred place of choice (where this is their home).
All our services are available at home, together with practical hands-on nursing care where needed.
